  • Schedule an appointment for all services including eye exams, contact lens exams, frame selection, eyeglass dispensing/pick up.

• LIMITED FAMILY/GUARDIANS
  • Come by yourself if possible. If you are unable, try and limit your party to one parent/guardian/helper.
• SYMPTOMS
  • If you are exhibiting any flu-like symptoms or recently come into contact with a COVID-19 positive individual prior to your exam, call our office to reschedule.
• INTAKE FORMS
  • New patients or returning patients with information changes, please print, fill out, and bring the following intake forms: 
    • Welcome Form 
    • Privacy Form 

When you arrive, please:

• FACE MASK
  • All patients and those accompanying them will be required to wear a face mask.
• WAIT FOR STAFF
  • Wait at the door when you arrive until a staff member signals for you to enter.
• TEMPERATURE
  • Your temperature will be taken when entering the building.
• WASH YOUR HANDS
  • We ask that upon entering our clinic, that you immediately wash your hands with soap and water or hand sanitizer.
• SOCIAL DISTANCING
  • We ask that you do your best to maintain social/physical distancing recommendations and keep a minimum of 6 feet between you, our staff, and other patients when possible.
